If you wish to submit an insurance claim form, please complete the appropriate claim form (as per instructions), have it signed by your Centre Secretary and ensure it is forwarded to the Little Athletics Tasmania Office - PO Box 812, Moonah TAS 7009. Willis Insurance do not accept Insurance Claims directly from individuals or centres.
